Award Criteria:
• INSTITUTIONS ARE LIMITED TO ONE NOMINATION PER YEAR FOR THIS AWARD
• A nominee must have been a woman student-athlete and a member of a NCAA sponsored varsity intercollegiate team during the current academic year.
• A nominee must have overcome significant personal adversity or disability/illness or have undergone extraordinary personal sacrifice during their journey to achieve recognizable success as a collegiate student-athlete. Economic and/or social background will not be considered. The following situations are excluded for award consideration: pregnancy, personal addictions e.g. alcohol or drugs. Common or typical athletically related injuries while not excluded from consideration are discouraged.

• A nominee must have evidenced a “significant” athletics contribution during the current academic year. Simply being a member of a varsity team is not sufficient for consideration.

Note 1: If the reason for the nomination is injury, illness or disease a letter from a medical provider must be attached electronically to this form.
Note 2: A letter from the student-athlete describing her journey may be attached to this form. This is not a requirement.

Note 3: Other supporting documentation may be electronically submitted with the nomination form, such as a student-athlete statement, but is not a requirement.
